Hopelink brokers Non-Emergency Medical Transportation (NEMT) in King and Snohomish counties for medical services covered by Medicaid.

Ride requests can be made between 8:00 a.m. and 5:00 p.m. Monday through Friday in either King or Snohomish County. Please call one the numbers below and have the following information readily available:

• Provider One Number
• Name of medical provider
• Name, address and phone number of medical facility
• Appointment date and time
• Reason for the appointment

Medicaid Transportation Document Submittal

This is a secure document submittal for documents like gas reimbursement information, lost Orca cards, and doctor’s form verifying a client needs a different mode of transportation. Please include the client’s ProviderOne number on the document being submitted and an email address that we can follow up with. To attach documents, simply use the select button.
