For records that aren't already available in MyChart – or if you don't use MyChart – you can request your records using any of the following methods you prefer. (For imaging requests, see the "Radiology Imaging and Reports" section below.)

We'll process your request within 15 days and send you the records using the method you indicated. You also may arrange to pick up your medical records in person at any of our Medical Records locations. Our office hours are 8 a.m. to 4:30 p.m., Monday to Friday.

Processing fee

We charge 25¢ per page to copy medical records or $25 if using an electronic record. We'll call to notify you of the total charge. You may mail us a check or pay the fee in person if you pick up your records.

Radiology imaging and reports

Your imaging test will be interpreted by a UCSF radiologist, a doctor trained to interpret imaging exams. The radiologist will write a report with the results of your exam. After the report is finalized – typically in two to five working days – there are two different ways you can access it and receive your results:

1. MyChart

The report and the images from your exam will be available in MyChart. This is the fastest way to receive your results. Please note: Because your results are available in MyChart immediately after they're finalized by the radiologist, your healthcare provider who ordered the study might not have reviewed them yet.

2. UCSF Imaging Library

The report and images are also available by CD, free of charge. To request a CD, complete the Authorization for Release of Health Information form (English) or Autorización de divulgación de información médica (Spanish).

Please include the following when you submit your form:

CDs are mailed five to seven business days after the release form is received.

If you have any questions about obtaining a copy of your images and report, call the Radiology Imaging Library at (415) 353-1640 (option 3). Hours are 7 a.m. to 6 p.m. seven days a week.

For help understanding your images and report, contact the provider who placed your imaging order.

Health information exchange

A health information exchange (HIE) allows your health care providers to use secure, electronic methods to share your most recent medical information with doctors' offices, hospitals, labs, radiology centers and other providers participating in your care. The purpose is to give each of your caregivers access to the most up-to-date information available.

Patients are automatically set to opt in to HIE, meaning information will be shared with participating physicians and health care providers unless you provide instructions to opt out. If you wish to opt out of participating in UCSF HIE, you can easily do so when checking in for your next visit to UCSF or by completing the UCSF Health Information Exchange Patient Opt-out Form and mailing it back to UCSF Medical Center Medical Records at:
